Segue Health -- Faith Based Medical Practice
Segue Health, a faith based medical practice, is seeking a compassionate Care Coordinator to join our team serving patients in the DeSoto county and surrounding areas coordinating post discharge transitional care management. This position requires a mix of clinical knowledge, excellent organizational skills, and excellent communication skills.
